Join us for a tour of PACE, the Program of All-Inclusive Care for the Elderly (PACE). A community-based program that provides 24-hour coordinated care for eligible older adults who want to stay in their homes for as long as possible. The PACE team works with each participant to develop a care plan that promotes independence and the highest quality of life while allowing choice and dignity for participants and their families. For ages 55 and over.
